Job Title: Licensed Social Worker

Location: Nursing Home in New Orleans, LA

Job Type: Full-Time

About Us: Our nursing home client in the heart of New Orleans is dedicated to providing exceptional care and support to our elderly residents. We are currently seeking a compassionate and skilled Licensed Social Worker to join our multidisciplinary team. This role is critical in enhancing the quality of life of our residents through comprehensive social services.

Key Responsibilities:

• Conduct initial assessments of new residents to understand their social, emotional, and psychological needs.
• Develop, implement, and review individualized care plans in collaboration with the healthcare team.
• Provide ongoing support and counseling to residents and their families, addressing concerns related to adjustment, mental health, and end-of-life planning.
• Act as a liaison between residents, families, and the healthcare team, ensuring effective communication and advocacy for resident needs.
• Coordinate with external agencies for additional services such as home care, legal assistance, and financial support.
• Organize and facilitate group activities and programs aimed at promoting social interaction, mental stimulation, and emotional well-being.
• Maintain detailed records and documentation in compliance with legal and ethical standards.
• Participate in interdisciplinary team meetings to discuss care plans and resident progress.
• Provide crisis intervention as needed and make referrals to appropriate community resources.
• Conduct regular follow-ups to assess the effectiveness of interventions and adjust plans as necessary.

Qualifications:

• Must hold a current license as a Social Worker in Louisiana.
• Master’s degree in Social Work from an accredited institution.
• Experience working in a healthcare setting, particularly in geriatrics or a nursing home environment, is preferred.
• Strong understanding of the social, emotional, and psychological challenges faced by elderly individuals.
• Excellent communication, interpersonal, and counseling skills.
• Ability to work collaboratively in a multidisciplinary team.
• Knowledge of community resources and services for the elderly.
• Proficient in maintaining accurate and detailed documentation.
