SUMMARY

Provides services to hospitalized members. Assists in developing, evaluating, and coordinating discharge plans and outpatient follow up.


DUTIES

Meets with hospitalized members/hospital social worker within 24-48 hours of admission to coordinate care and discharge plan.

Requests hospital records/Title 36 petitions.

Schedules/facilitates ART/CFT within 72 hours of admission.

Coordinates with OP clinic at admission, during hospitalization and at discharge.

Assists in coordination of services to prepare member for discharge to include 7-day BHMP follow-up appointment.

Coordinates transitions to alternate levels of care.

Facilitate BHMP-to-BHMP reviews if the inpatient treating BHMP does not agree with discharge plan.

Complete SMI determinations as appropriate.

Complete urgent enrollments as requested by hospitals.

Coordinates with COT liaison for members on petition/court hearings.

Complete DCS Rapid Response assessments.

Completes assessment/treatment plan as necessary.

Documents all clinical contacts in electronic health records.

Participates in on-going clinical supervision and ensures that four hours are received each month.

Attends regular team meetings as required.

Performs all other duties as assigned.

REQUIREMENTS

Bachelor’s degree from an accredited college or university in a behavioral health related field

Bachelor’s degree in any field plus one year of work experience in behavioral health service delivery

Associate’s degree in a behavioral health related field from an accredited community college institution and two years work experience in behavioral health service delivery.

Associate’s degree in any field from an accredited community college or institution and four years work experience in behavioral health service delivery.

HS diploma/GED and four years work experience in behavioral health service delivery.

Knowledge, Skills, Abilities

Demonstrated competency in Managed Care systems and provider networks.

Demonstrated competency in the adult recovery/child family teams models.

Knowledge of community and agency resources.

Demonstrated competency in the philosophy of behavioral health programs.

Ability to work independently.

Demonstrated competency in clinical assessment.

Ability to be flexible.

Above-average verbal and written communication skills.

Other Requirements

Reliable transportation, Valid AZ driver’s license, automobile insurance, good driving record.

Proof of physical Exam and TB skin test
