Job Title: Inpatient Pharmacist Night Lead

Location: Burlington, VT 05401.

Job Type: Full-Time

Salary: $125,299.00 - $187,990.00 per year

Signing Bonus: $3,000 - $5,000 (based on experience)

Relocation Package: Partial

Job Description:

We are seeking a dedicated and experienced Inpatient Pharmacist Night Lead to join our multidisciplinary care team. The ideal candidate will be responsible for providing high-quality pharmaceutical services, including monitoring patient drug therapies, interpreting medication orders, and offering drug information to patients and healthcare professionals. The position involves working closely with support staff and overseeing pharmacy operations during the night shift.

Key Responsibilities:

  • Provide pharmaceutical care to patients by verifying orders, monitoring drug therapies, and conducting medication histories.
  • Collaborate with the healthcare team to optimize medication use and ensure patient safety.
  • Perform core clinical activities, including pharmacokinetics, IV to PO conversions, and therapeutic interchanges.
  • Assist in leading a team of pharmacists during night shifts, ensuring efficient pharmacy operations.
  • Participate in antimicrobial stewardship and medication safety initiatives.

Qualifications:

  • Doctor of Pharmacy Degree (preferred) or Bachelor of Science Degree from an ACPE accredited School of Pharmacy.
  • Active pharmacist license in good standing in the State of Vermont.
  • At least 1 year of pharmacist experience in a health system required.
  • Night shift experience and residency training are preferred.
  • Familiarity with EPIC and inpatient pharmacy practices is a plus.

Work Schedule:

  • Night shifts, with rotation through weekends and holidays.
