This is a 13-week special project with a strong likelihood of extension or the chance to join our client full-time!

What You'll Do:

Provide personalized and compassionate care to patients, working primarily in a virtual environment.
Conduct in-home assessments (comprehensive annual wellness exams, focused screenings, etc.) as needed within a specific geographic area.
Complete 3+ patient visits daily.
Counsel and educate patients and families about available benefits and programs to support healthier living.
Meticulously document patient information (chief complaint, diagnoses, history, reviews, exams, medications, allergies, assessments, and plans).
Complete all documentation promptly.
Maintain high-quality care standards.
Assist patients in accessing educational resources.
Participate in integrated care team meetings.
Assess patients' health status.
Develop and implement care plans.
Follow up and evaluate patient progress.
Perform other duties as assigned.
What You'll Need:

Active and unrestricted Registered Nurse license.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with required medical instruments and equipment.
Knowledge of computer-based data management programs and information systems, medical records, and point-of-interview technology.
Excellent verbal and written communication skills to interact effectively with partners, patients, families, physicians, and community representatives.
Sound understanding of federal and state regulations, including HIPAA and OSHA.
2+ years of direct patient care experience.
